Writing a care note

Write what you saw and did for that resident. If your home keeps note templates, you can start from one and fill it in by tapping.

Write a note

  1. Open the resident and tap Care notes, or open Care notes in the menu and tap Add care note, then choose the resident.
  2. Choose the Type of note, such as Shift note or Change in condition.
  3. Write what you saw and did. Use the resident’s name, not initials.
  4. Tap Save care note. MapSorix Care says who it was saved for and when.

A note is signed with your name and can’t be deleted. To fix one, see Correcting a mistake.

Start from a template

A template is wording your home has written for notes you write often, such as meals or a shift note. You fill it in for this resident, so it takes less typing.

  1. On the note form, tap Start from a template and choose one. Templates of the type you chose come first.
  2. For each choice, tap the one that is true. Tap it again to take it back.
  3. Type a few words in each blank.
  4. Add anything else in the box below the template.
  5. Tap Save care note. It stays off until every part is filled in, and says how many are left.

Want to change a word the template gave you? Tap Edit as text and the note becomes ordinary text you can change. Don’t want the template after all? Tap Remove template; what you typed below it stays.

The note is still yours: it’s saved with your name, like any other note. Only fill in what is true for this resident today.

Managing templates (nurses and administrators)

  1. Open Care notes and tap Note templates. Administrators can also open it from Settings, under Care note templates.
  2. Tap New template, give it a name and a type, and write it.
  3. Put a choice in square brackets, with the options split by slashes: Fluids: [good / fair / poor].
  4. Put a blank as three underscores, ___, or a short hint in square brackets: Checked at [time].
  5. Try it under What care aides will see, then tap Save template.
  • Every template needs at least one part to fill in. A template with nothing to fill in would save the same note every time, so MapSorix Care won’t save one.
  • Add starter templates offers starter templates (shift note, appetite and fluids, sleep, skin, mobility and transfers, behaviour, family communication and change in condition). Add the ones you want, then adapt their wording to your home. Have a nurse read each one before care aides use it.
  • A template you no longer want is retired, not deleted, and can be used again. Changing a template never changes notes already written from it.
  • Don’t put a resident’s name or health details in a template: every staff member in the home sees it.
